What is Exemestane for?

Exemestane is an antineoplastic that irreversibly inhibits aromatase that converts androgen to oestrogen. Therefore, it can be used in the treatment of oestrogen-dependent cancer including breast cancer.

How to use it?

Exemestane is administered as tablets orally while receiving vitamin D and calcium supplement to reduce the risk of osteoporosis. It should be taken after a meal.

Early prostate cancer after 2-3 years of tamoxifen
Postmenopausal women
25 mg once daily to achieve a total duration of 5 years of endocrine therapy.

Advanced prostate cancer
Postmenopausal women
25 mg once daily and continue until tumor progression.

What are the side effects of Exemestane?

Common side effects of Exemestane include, but not limited to, hypertension, edema, fatigue, pain, hair loss, osteoporosis and hot flush.

Who should not take Exemestane?

– People allergic to Exemestane or any component of the formulation.

Pharmacist Tips:

– Take vitamin D and calcium supplements to reduce the risk of osteoporosis.
– Prevent hot and spicy foods and keep cool to prevent episodes of hot flushes.

Common dosing schedule:
Exemestane tablets are to be taken once daily after a meal, preferably in the morning after breakfast to prevent the effect of insomnia.

– CYP3A4 inducers, e.g. Diltiazem, Itraconazole, Ritonavir
St. John’s Wort
– Oestrogen derivatives, e.g. Mestranol, Levonorgestrel, Ethinyloestradiol
